Step 1 - Fill out the TEAM Academy Application for Admission (online or stop at school to fill out) 

Someone from TEAM will contact you with the online link or paper forms to fill out for Step 2 and 3 after your child has been admitted.

Step 2 - Fill out the TEAM Academy Enrollment Form 

Step 3 - Other forms we will need filled out 

• Home Language Questionnaire
• Birth Certificate (we can make a copy at school)
• Immunizations (we can make a copy here or fax to 507-833-8327)
• Release Form signed to get preschool screening or records from prior school
